Treatment of POEMS syndrome with bevacizumab.

چکیده
We report a case of POEMS syndrome which relapsed six years after autologous peripheral blood stem cell transplantation. According to encouraging data published recently, we treated the patient with cyclophosphamide, dexamethasone and the VEGF-antibody bevacizumab. After an initial improvement, the subsequent course was complicated by severe adverse events leading to multiorgan failure and death. This dramatic decline highlights the need for further investigation before using bevacizumab in patients with POEMS syndrome.

Polyneuropathy, organomegaly, endocrinopathy, monoclonal protein, skin changes (POEMS) syndrome refractory to complete serum vascular endothelial growth factor (VEGF) blockade: insights from sequential VEGF monitoring
Polyneuropathy, organomegaly, endocrinopathy, monoclonal protein, skin changes (POEMS) syndrome is a systemic condition related to plasma cell dyscrasia. Increased vascular permeability is responsible for some of the hallmarks of this disorder that may include renal microangiopathy. Several lines of evidence suggest that vascular endothelial growth factor (VEGF) is central to the pathogenesis o...
متن کاملBevacizumab therapy before autologous stem-cell transplantation for POEMS syndrome.
POEMS is an acronym for a rare multisystemic syndrome encompassing polyneuropathy (P), organomegaly (O), endocrinopathy (E), monoclonal gammopathy (M), skin changes (S) and additional features variably expressed [1]. POEMS syndrome is associated with high vascular endothelial growth factor (VEGF) serum levels. This angiogenic factor likely plays a pathogenic role, in particular for neuropathy, ...

PURPOSE To report our experience in managing a case of bilateral optic disc oedema and unilateral cystoid macular oedema with a posterior subtenon (PST) injection of triamcinolone acetonide and intravitreal bevacizumab (IVB) in a patient with polyneuropathy, organomegaly, endocrinopathy, monoclonal gammopathy, and skin changes (POEMS) syndrome. CASE REPORT A 50-year-old healthy woman complain...
متن کاملVascular endothelial growth factor promoted endothelial progenitor cell mobilization into the peripheral blood of a patient with POEMS syndrome.
We assessed the percentage of endothelial progenitor cells (EPCs) in the peripheral blood of a patient with POEMS with elevated VEGF plasma levels. High VEGF plasma levels were associated with increased EPC concentration and treatment with an anti-VEGF antibody induced a consensual decrease of both parameters. In vitro cultures of the patient BM cells suggested that the stromal compartment coul...
متن کاملOptic Disc Edema Responding to Localized Anti-vascular Endothelial Growth Factor Treatment in a Patient with POEMS Syndrome
Dear Editor, Although various underlying conditions such as increased intracranial pressure, inflammation, ischemia, or compression can induce optic disc edema (ODE), all forms of disc swelling ultimately result from blockage of axo-plasmic transport [1]. In contrast to these types of ODE, which can generally be managed by systemic treatment of the underlying etiology, we describe a case of ODE...
